Why have I been asked to do this?

Your hospital has asked you to fill in this assessment to help get you ready for your upcoming procedure.

We will ask you questions about you, your health, and your procedure. The answers you give to these questions will help your hospital to plan your care.

By completing this assessment online, you may not need to come to the hospital for a pre-procedure assessment. Once completed, your healthcare information is sent directly to your hospital team.

Where can I find my code?

Your code can be found in the SMS or email sent to you by Ultramed. If you can't find your code, please get in touch with your hospital team.

What if I am unable to complete the assessment myself?

A person you trust can complete this assessment for you if you give them your permission. This could be a family member or carer.

Do I have to do this in one go?

You can come back to your pre-procedure assessment if you don't complete it in one session. You will need your 12 character access code to continue with your assessment.
